ABSTRACT

Objective: To determine frequency of Minimal Hepatic Encephalopathy in illiterate patients with compensated cirrhosis


Methods: Illiterate patients with compensated cirrhosis with F4 Score on Shear-wave Elastography were selected for study after informed consent. Sample size was estimated at 106. Selected patients were subjected to two tests for detection of MHE, Number Connection Test A and Block Design Test. Patients taking ? 30 seconds were labelled as Positive for MHE


Result: Out of 110 selected patients 10.9% were alcoholics and in 8.2% of patients no hepatic virus infection was detected. HCV was positive in 48.2% patients while HBV was positive in 13.6% of patients. MHE was detected in 72 [65.5%] of patients. Major differences were found in MHE Stage II and III by two tests. Over all BDT detected more cases and gave higher Staging in Stage II and III as compared to NCT-A test


Conclusion: Minimal Hepatic Encephalopathy [MHE] could be detected in illiterate patients using NCT-A and BDT Tests

ABSTRACT

The objective of this study was to provide a baseline data on the pattern of oral squamous cell carcinoma in Karachi and to find a correlation between the clinical patterns of oral squamous cell carcinoma with age and site. The retrospective cross sectional study was conducted. The patients of oral squamous cell carcinoma were examined from January 2007- December 2010 at oral maxillofacial department, Darul Sehat Hospital Karachi. All retrievable case files were obtained and necessary data were extracted regarding age, gender, site and histological type. All cases were clinically examined and provisionally diagnosed. Biopsy was taken from the lesions and tissues were fixed in 10% buffered formalin and submitted to histopathologic department for histological confirmation. 62% of males and 38% females were diagnosed with squamous cell carcinoma. 30% were observed in age group of 41-50 years old. 45% of squamous cell carcinoma were on buccal mucosa. Correlation between the 2 variables i.e., site to histological type and age to histological type was found to be statistically significant. Level of significance was set at 0.05. However, the strength of association was weak between the selected variables. OSCC was most common of all oral malignancies of which most were in the older age group. Serious thought should be given to prevention and early detection

ABSTRACT

To determine the frequency of Pott's disease in patients of paraparesis presenting to medical wards of Civil Hospital Karachi. Cross sectional study. This study was conducted in all Medical Wards of Civil Hospital, Karachi, from 1[st] January 2012 to 31 Dec 2012. A total of 133 patients of spastic paraparesis and meeting inclusion criteria were included in this study. Inquiry was carried out regarding age, gender, presenting complaints, duration of illness, past history of TB and site of involvement of spine. Diagnosis of Pott's diasease was made on Magnetic Resonance Imaging [MRI] findings of soft tissue edema, disc space narrowing, paraspinal mass or vertebral collapse. Out of 133 patients, mean age was 47.7 [ +/- 15.6] years with male: Female = 1.6: 1. Out of 133 patients of spastic paraparesis, Pott's disease was diagnosed in 53 [39.8%] cases. Frequency of pott's disease was high in male [M: F = 2.1: 1], 36 [43.9%]. Average age of patients with pott's disease was 49 +/- 13.2 years [Min - Max = 22 - 80 years]. In this study Pott's disease was diagnosed in 39.8% patients of spastic paraparesis. Frequency of pott's disease was high in male gender and older [Age: 31 - 50 years] patients

ABSTRACT

To determine the frequency of dyslipidemias in patients with stroke and to compare the lipid levels in ischemic and hemorrhagic stroke patients. An observational study. Medical wards of Civil Hospital Karachi. January 2010 to June 2011. A total of three hundred and fifty- five patients above the age of 18 years with clinical evidence of stroke. CT scan brain and fasting lipid levels were done in all patients. The lipid levels were compared between the ischemic and hemorrhagic stroke patients. Data was analyzed with SPSS version 16.0. Out of the 355 patients 161 [45.4%] had dyslipidemia. Among the dyslipidemic patients, there was not a statistically significant difference in serum cholesterol, triglycerides, and LDL and HDL levels between ischemic and hemorrhagic stroke. [p- value > 0.05]. There is a high frequency of dyslipidemia in stroke patients. While comparing ischemic and hemorrhagic strokes there was not a significant difference between the various lipid fractions.
